Takes a List of Tuple (as Object[] or entities) from a ResultSet. Default implementation assumes a TUPLE1 as Object[3]: {@code <Name1>,<Display Name 1>,<Parent Category Name 1> } or Object[2] <Name1>,<Display Name 1> or Object[1] <Name1>
If there are 3 fields in the Tuple and the 3rd is not null then a parent is assumed or else a tuple is assumed not to have a parent - This also results in a FLAT list
Note: Menu MenuItem.names are case insensitive while comparing.
Any parent tuple should appear before its child tuples in the list; or better yet let this method determine the parents TODO: Try and remove this restriction of dependence on Order for pre-passed parents
